Human Papillomavirus (HPV)

Women diagnosed with high-risk Human Papillomavirus (HPV) face a series of psychosexual and emotional challenges.

The sexually transmitted nature of HPV generates feelings of concern, surprise, self-blame, stigma, shame, and confusion. Many women try to understand the source of their infection, which can lead to trust issues and infidelity in their relationships.

Additionally, there is uncertainty about the possibility of transmitting HPV to sexual partners, causing guilt and worry. The impact on sexual relationships includes a decrease in interest and frequency of sexual relations, and in some cases, complete cessation of sexual activity.

Disclosing the infection to partners, friends, or family is described as uncomfortable and embarrassing. It is crucial to have a cervical pathology specialist who doesn't judge and can provide clear information and emotional support.

Dr. Belda helps women understand the high prevalence and asymptomatic nature of HPV, as well as its ability to clear spontaneously. We know that a compassionate and non-judgmental approach by the doctor can mitigate adverse psychosexual consequences and help women better manage their diagnosis and interpersonal relationships.

Colposcopy

The test known as colposcopy is considered an essential tool in the secondary prevention of cervical cancer.

It is an examination in which, using a colposcope, the cervix is examined in real-time. This way, it allows identifying cervical intraepithelial lesions, knowing their location, extent, and characteristics, as well as taking biopsies to obtain diagnostic confirmation and decide whether the patient can benefit from surgical intervention, vaginal laser intervention, medical treatment, etc.

Endometriosis

Endometriosis is a chronic inflammatory disease that affects 8-10% of women of reproductive age and 30-50% of women with infertility.

It is characterized by the presence of endometrial-like tissue outside the uterus, causing severe pelvic pain, dysmenorrhea, dyspareunia (pain during sexual intercourse), and dyschezia (pain during defecation). This condition significantly impacts women's daily lives due to its emotional and economic costs, which can lead to deteriorated psychological health.

Intimate relationships are affected by endometriosis, not only due to infertility but also because of its negative consequences on social life, work, intimacy, and sexuality. Dyspareunia is a major cause of sexual dysfunction, reducing or eliminating sexual activity, which negatively affects self-esteem and intimate relationships.

Managing endometriosis is a complex process that requires actions related to the disease, daily life, and reviewing future plans. The disease has a negative impact on intimate relationships and women's psychological health. Relationship satisfaction and effective coping with the disease are associated with better psychological conditions.

In this sense, early diagnosis allows initiating appropriate treatments that can reduce pain and improve quality of life in women suffering from this condition.

Menopause

Many patients come to Dr. Belda seeking answers about menopausal hormone therapy (MHT) and its impact on daily life.

Menopause is associated with various symptoms that can significantly deteriorate quality of life, such as musculoskeletal pain, fatigue, mood disorders, sleep problems, irritability, vaginal dryness, and sexual disorders. These symptoms affect work capacity and personal relationships, in addition to increasing demand for medical services and work absenteeism.

MHT, although popular in the past, was abandoned in 2002 after studies indicated more risks than benefits. However, MHT can improve quality of life by reducing the intensity of climacteric symptoms.

Additionally, menopause is related to increased adiposity and risk of chronic diseases, such as obesity and depression, due to changes in body neurochemistry. MHT not only relieves vasomotor symptoms but also improves other climacteric symptoms, positively impacting quality of life.

At Clínica Belda, we consider it crucial to provide unbiased information about menopause and consider MHT to improve the quality of life of affected women, always taking into account the risk-benefit balance for each patient.

Polycystic Ovary Syndrome (PCOS), Menstrual Disorders, and Infertility

Polycystic ovary syndrome (PCOS) is a condition characterized by irregular or absent menstrual periods, obesity in some cases, and symptoms caused by elevated male hormone levels (androgens), such as excess body hair and acne.

The exact causes of PCOS are unknown, but it is believed to be related to a malfunction of the enzyme that controls androgen production. Women with PCOS may experience insulin resistance, which increases the risk of diabetes and metabolic syndrome. Additionally, PCOS is the most frequent cause of infertility in young women in our country.

PCOS symptoms usually appear during puberty and may include irregular menstrual bleeding, acne, hirsutism, weight gain, and acanthosis nigricans. Diagnosis is based on symptoms, blood tests to measure hormone levels, and ultrasounds to detect ovarian cysts.

PCOS treatment has been the subject of debate for years. At Clínica Belda, we start from a fundamental premise: "Not all treatment options involve hormonal contraceptives!"

Within the framework of polycystic ovary syndrome, as in all menstrual period disorders, there are dozens of effective and safe alternatives with scientific evidence that can not only help regulate the cycle but also cure the root of the problem.

These include strength exercise, body composition changes, endocrinological medication such as metformin and spironolactone, and specific treatments for excess body hair and acne. Women who wish to become pregnant can benefit from medications such as clomiphene and metformin to stimulate ovulation.

Gynecological Oncology

Probably one of the worst words a woman can hear: Cancer. Your world falls apart, you're invaded by fear, insecurity, uncertainty... and all of this immersed in a sea of doubts and sadness that can completely condition how you face this new situation.

We are committed to helping and supporting you through this difficult process, with availability to resolve doubts and advise you on often overlooked issues such as diet, oncological aesthetics, sexual consequences of treatments, or the psychological dimension: much affected and ignored.

Contraception

Not all contraceptive options involve oral medications! We emphasize this point because it's a crucial reality in modern gynecological care.

An increasing number of women are choosing not to pursue motherhood, and we fully support this personal decision. However, many women face the consequences of unintended pregnancies due to insufficient information, which deeply concerns us: we aim to prevent anyone from experiencing the emotional trauma of making difficult decisions in such situations.

Dr. Belda, holder of the Expert Title in Contraception from the University of Alcalá, provides personalized counseling to recommend the most suitable contraceptive method based on your individual needs, lifestyle, and priorities: From subdermal implants to intrauterine devices (IUDs), we offer various options including transdermal patches, vaginal rings, oral contraceptives, and for those certain about their reproductive choices, tubal sterilization. Our goal is to provide comprehensive contraceptive care with multiple options.

Sexually Transmitted Infections (STIs) and Vaginitis

Receiving a diagnosis of a Sexually Transmitted Infection (STI) can be challenging, and it's even more difficult when the healthcare provider doesn't create a safe and supportive environment.

We assist, advise, and accompany based on scientific evidence with the intention of reducing the consequences for your health. Holder of the STD EXPERT title accredited by the University of Alcalá de Henares, Dr. Belda can help you in the treatment of sexually transmitted diseases and recommend means to reduce reinfections.

Vaginitis and Bartholin's gland abscesses are one of the most frequent reasons for consultation in gynecology. The desperation of many women who suffer from these conditions repeatedly, who can't find the right treatment, has encouraged Dr. Belda to delve into the ideal management of these cases.

In tune with expert nutritionists, microbiologists, and pathologists, we will try to find the key so you don't continue suffering. The specific management of Bartholin's gland abscesses can be cumbersome, as they are usually recurrent, painful conditions with painful surgeries as the main cure.

Genitourinary Syndrome of Menopause (GSM) and Vulvar Lichen Sclerosus

These are two distinct conditions that are frequently confused, but they require different approaches to treatment!

The management of Genitourinary Syndrome of Menopause (GSM) focuses on rehydration and restoration of the superficial layers of the genital mucosa. When properly identified, it can be effectively managed. Clinical presentation includes recurrent urinary tract infections, vaginal dryness, pruritus, burning sensation, dyspareunia, and physical changes on examination such as decreased labia minora thickness, mild mucosal pallor, or vaginal stenosis, among other symptoms.

From hormonal moisturizers, laser therapies, hyaluronic acid, oral hormone therapy with Ospemifene, MHT, or Platelet-Rich Plasma, the management of this condition must be done with good counseling and frequent follow-up.

Lichen sclerosus, however, is an entity that, although it mimics the symptoms of menopausal atrophy, is due to an autoimmune condition and can also affect young women. The management is more complex, tedious, and requires vulvoscopic follow-up. The therapeutic approach should be done with corticosteroid medication, immunosuppressants, and moisturizers at high doses, although in many cases it may require more aggressive management to correct the physical ravages of this disease, at which point your doctor should consider the use of stem cells to restore your anatomy.

In tune with plastic surgeons, the counseling of your case should be analyzed and evaluated by experts in Vulvar Pathology, and in our clinic, you can trust that the advice we give will always prioritize your physical and psychological well-being.
